Error Found in the Custom UI XML of "Microsoft Dynamics CRM"

I got this error during outlook offline in MS CRM 2011.Ribbons were working fine in the Online mode, whereas it failed to load in outlook offline.

Later on i came to know that main cause of the error was the duplication of the  CustomAction ID which I Just copied and paste for adding a custom button on some entities.

It was working for online because in online ID's are generated  on the fly whenever the ribbon loads.But It is not the case with Outlook offline.

It happens whenever we customize Home ribbon in MS CRM 2011 .
